Author Consent Form

You, as the Author (or Signee, if different), agree to the following terms:

1. Approval Process: You will receive two proofs (one digital and one hard copy) for review and approval. By giving your final approval, you confirm that there are no issues or conflicts, and you take responsibility for the accuracy and final content of your Life Book. This approval grants us the authority to print.

   
   

2. Content Ownership: You retain ownership of the intellectual property in any materials (text, photos, recordings, etc.) you submit to us while granting us the rights to use them for our services and marketing materials.

   
   

3. Personal Opinions and Indemnity: Any opinions in the Life Book are your own. You agree to indemnify Legacy Life Books LLC and it’s Associates from any claims, losses, or liabilities related to the content of your Life Book.

   
   

4. Consent for Personal Data: You consent to the transcription of your interviews and processing of any sensitive personal data you disclose, which will be used solely for documenting and publishing your Life Book. Personal information is processed under applicable data protection laws.

   
   

5. Liability: Legacy Life Books LLC is not liable for any reason whether direct, indirect, special, or consequential losses related to the project. 

   
   

6. Responsibility for Third-Party Information: You accept responsibility for any personal information about third parties included in your story.

   
   

7. Copyright and Publication: You give copyright permission to Legacy Life Books LLC to publish your Life Book.
